Once you have faxed or uploaded your health screening form it can take a few days for this to show up in our system and then in your account. If it has been more than 2 weeks since you sent us your form chances are, we never received it or it couldn't be processed.
The best thing you can do is resubmit your form at www.adurolife.com/submission. You will receive an email confirmation of receipt that you may save for your records.
*PLEASE NOTE: ADURO is not responsible for forms that cannot be read or that are not received due to fax technical issues. It is each member's responsibility to track and follow up on any forms or information sent to ADURO. We recommend tracking it until you see it in your program. Resubmitting your form could mean that you will miss your deadline. Depending on how your program is set up, you most likely have a "Lookback Date." As long as your screening was done within that date range your form will be accepted, UNLESS your program has a Submission Deadline, the date in which all forms must have been sent to ADURO.
