Depending on your company's program design you may be able to go to your physician and complete the screening requirements with them. You will need to log into your member platform and download the "HPSF" or Health Provider Screening Form which can be found in your "COMPLETE HEALTH SCREENING TILE."
Download and print this PDF to take to your primary care provider. NOTE: everyone has a customized form with pre-filled out with individual details. You can't share your form with anyone else.
*Completing your Health Provider Screening form
- Have your health provider complete and sign the form.
- Submit the signed form via one of two secure methods (either you or your health provider's office can complete this step):
- Upload at forms.wellmetrics.life/submission
- Fax to 1-866-877-7983.
- Note: Please do not include the cover page of the form in your submission. Only page 2 is required.
- Keep a copy of the successful transmission for your records.
It can take anywhere from 72 hours to 1 week to see these results populated in your account and credit applied.
