We are only able to accept health data from participants through direct file feed from an approved and integrated health provider, through onsite screenings, or via online upload of the approved ADURO health screening form.
Health data that that is electronically received is processed via secure portals meeting all compliance and security standards. These portals can only read a specifically formatted documents that are available through your wellness program.
If you have a form that you would like to send to us you may:
Option 1) fax the form to 866-877-7095. Please note this option will take about 72 business hours to process and we are not able to confirm receipt until the form is processed.
Option 2) Digitally scan the form to your computer or take a picture with your phone, and then upload it to our secure website at adurolife.com/submission. When you upload it to our website you will receive immediate confirmation it has been received.
