Due to HIPAA privacy laws, we are unable to discuss specific health screening results via email. However, if you would like to know more about the healthy ranges for the values submitted for the health screening requirement, you can log onto your wellness account page, click on 'My Points' section on the top of your page and then scroll down to healthy measures section.
Use the 'learn more' drop down to the right of each healthy measure. Here you will find information about your program's healthy measure requirements. If you meet the health standards, you will automatically earn points.
You can update this result with a better result at anytime during your program if you'd like to be awarded these points. Simply submit a new Health Provider Screening form.
Because you have already had a screening you'll find this form under the HISTORY section of your program page. Submission instructions are located at the top of the forms for your convenience.
Option 1) is to fax the form to 866-877-7095. Please note this option will take about 72 business hours to process and we are not able to confirm receipt until the form is processed.
Option 2) Submit via the secure online upload~ you can scan the form to r computer or take a picture with your phone, and then upload it to our secure website at adurolife.com/submission. When you upload it to our website you will receive immediate confirmation it has been received.
